Emergency conditions in dental practice
Lvov, 2004
In dental practice, as in other branches of medicine, a doctor must have not only deep specialized knowledge and practical skills, but also be able to diagnose emergency conditions in patients and victims, while providing the necessary medical care. Exactly this current problem devoted to this work experienced clinical scientists. Although the issue of emergency care is considered in the curricula of higher education and postgraduate training, there is a need to publish just such a manual - small in volume, but rich in the most necessary material, sufficient for dentists (and not only dentists) to provide first medical care at emergency conditions, which, unfortunately, still quite often arise during the patient’s stay in the dental chair. Unfortunately, dentists do not always know how to consistently and competently help a patient in extreme situations, in particular in terminal state, even before the intervention of an intensivist-resuscitator. The manual addresses issues of providing emergency care to patients in extreme situations that arise during treatment. The manual is recommended for dental students and dentists

In the textbook on modern level all main sections are presented therapeutic dentistry, the main stages of its development are described. Current data on the etiology and pathogenesis of non-carious lesions, caries and its complications, periodontal diseases and oral mucosa are presented. Covered in detail latest achievements in the field of their diagnosis, treatment and prevention. Modern technologies of dental restoration and methods of endodontic treatment are described in detail. Achievements presented recent years on theoretical aspects of therapeutic dentistry, as well as modern methods of diagnosis, treatment and prevention of periodontal diseases and oral mucosa.

PREFACE

Improving the quality of dental care for the population is currently the most important task. Existing syllabus, according to which 2800 hours are allocated for dentistry, allows you to master the basics of the specialty.

One of the factors contributing to the development of a specialty is textbooks. This textbook outlines modern approaches to the main problems of therapeutic dentistry. In the section devoted to diseases of the oral mucosa, the connection of this pathology with the state of the body is emphasized, since the first manifestations of some common diseases(blood diseases, sexually transmitted diseases, including AIDS, etc.) can be in the oral cavity. The training of a future dentist in therapeutic dentistry can be called basic for two reasons. Firstly, the bulk of the population applying for medical care, are patients with diseases of the teeth, periodontium and oral mucosa. IN last decades specialization was carried out, as a result of which therapeutic, surgical, orthopedic dentistry, pediatric dentistry, and maxillofacial surgery were formed. That's it. At the same time, the transition to new forms of management and the widespread introduction of a market economy in dentistry is accompanied by an expansion of the service sector with individual look activities, the demand for specialists increases significantly generalist, i.e., a doctor capable of providing assistance with treatment and prosthetics. At the same time, there is a need for any doctor to

everyone modern methods treatment, and also knew how to treat children.

The urgent objectives of the learning process are the following:

▲ mastery of methods for preventing major dental diseases and the ability to implement them in everyday work;

Examination of the child. Outpatient card. Recording temporary and permanent survey data. Purpose of the lesson. Learn to correlate the clinical and radiological picture with the stages of formation. Questions and tasks to control initial knowledge 1. Why pediatric dentist should know the stages of tooth development? 2. List the stages of tooth development and parts of the tooth germ. 3. Indicate the timing of the formation of the dental plate. 4. What harmful factors and in what time frame can have an adverse effect on various stages tooth formation? 5. What signs is it characterized by? normal process teething? Scheme of the approximate basis of action: tooth development in the clinical and radiological aspect Data x-ray examination Stages of tooth development Period of initiation and formation of the germ 1. Formation of the dental plate in the 6th 7th week of the prenatal period 2. Differentiation of the cellular elements of the temporary tooth germ Not carried out In utero not carried out Intraoral examination data The reason for an x-ray examination may be the absence of eruption during the period or the presence of supernumeraries

4 6 6th semester 3. Histogenesis (building of dental tissues) from the 4th month intrauterine development, odontoblasts build dentin; enameloblasts enamel (first organic matrix, then primary mineralization occurs) Root condition 1. Unformed root 2. Stage of unformed apex 3. Stage of unclosed apex The dental follicle is revealed in the form of a rounded center of clearing with a clearly defined rim of the cortical plate along the periphery. The mineralization process begins from the top of the tooth crown and spreads towards the neck. Initially, pinpoint calcifications appear along the cutting edge and cusps, which merge to form a ring, cap, and contour of the tooth crown. A growth zone protrudes into the crown, limited along the periphery by a compact plate. The root is short, the walls are thin, almost parallel, turning into a bell, limited by a rounded area of enlightenment with a clear cortical plate along the periphery. The root is almost normal length, having a bell, the germinal zone is significantly reduced in size The root is of normal length with a pointed apex, the apical foramen is wide The reason for an X-ray examination may be the absence during the period of eruption or indications for removal of temporary The reason for an X-ray examination may be pulpitis, periodontitis, tooth trauma during the period of unfinished root formation Indications are the same Indications are the same

5 Lesson 1. Development of the tooth in the clinical and radiological aspect 7 4. The root is formed The root is of normal length with a pointed apex, the apical foramen is narrow. The periodontal gap at the apex remains widened for another 1.5 to 2 years 5. Resorption of temporary roots Roots are shortened, edges are eaten away near the follicle permanent tooth The indications are the same. The mobility of temporary II-IV degrees is determined. Scheme of the approximate basis of action: pathology of the formation of rudiments Stages of tooth development 1. Formation of the dental plate: a) hypofunction of the dental plate b) hyperfunction of the dental plate X-ray data are not determined in utero. Most often examined after a year. On the radiograph, the absence of individual rudiments or, less often, of all rudiments is noted. An increase in the number of dental rudiments, fully formed, of normal or unusual structure, is determined. Fusion is determined only in the area of crowns or roots, and sometimes crowns and roots together. They may have a common dental cavity. Data from intraoral examination. Occurs in temporary and permanent dentition. Complete primary adentia is rare and may be a manifestation of hereditary ectodermal dysplasia. Partial primary adentia is more common 1.2, 2.2, 3.5, 4.5 Usually discovered after eruption. Supernumerary teeth are located in or outside the dental arch. They may have a normal or modified shape (spike-shaped, conical, etc.). In temporary, less often in permanent dentition, fused, fused, twin teeth are found. More often they grow together

6 8 6th semester 2. Differentiation of cellular elements 3. Histogenesis, initial mineralization: a) abnormal development one tooth germ (simple odontoma) b) abnormal development of several tooth germs (complex odontoma) c) hereditary pathology: amelogenesis imperfecta, dentinogenesis imperfecta, odontogenesis imperfecta (Stainton Capdepont syndrome) or separate. One of the fused ones may be unerupted. Sometimes in the form of a correctly formed tooth, but with atypically located hard tissues inside the crown or root of a tooth Conglomerate or tooth-like structures at different stages of formation in the form of a dense heterogeneous shadow of irregular round and oval shape with clear scalloped contours A significant part of the crown has increased transparency, pathology from the roots is not noted The presence of short and thin roots, obliteration of cavities and canals in varying degrees, depending on the degree of formation, both temporary and permanent, the first with the second, the second with the third, the supernumerary with the complete one of the same name a) On upper jaw teeth in anterior section and molars of normal shape and size, on the lower jaw at the molars. More often 1.2, 2.2 are normal or increased in volume b) Often discovered by chance during an x-ray examination. Plaster, grooved teeth, desquamation of enamel, etc. are detected. With dentinogenesis imperfecta, the size, shape and color of the crowns are normal. With imperfect odontogenesis, opalescent color, abrasion of enamel and dentin up to the necks without exposing the tooth cavity
