Orlistat is a powerful and specific inhibitor of gastrointestinal lipases with a long-lasting effect. Its therapeutic effect occurs in the lumen of the stomach and small intestine and consists of the formation of covalent compounds with the active serine site of gastric and pancreatic lipases. The inactivated enzyme then loses its ability to break down food fats that come in the form of triglycerides and affect free fatty acids that are absorbed and monoglycerides.

Pharmacokinetics.

Suction.

A study involving normal weight and obese volunteers showed that the effect on absorption levels was minimal. After 8:00 after oral administration of the drug, unchanged orlistat could not be determined in the blood plasma, which means that its concentration is below the level of 5 ng/mol.

Distribution.

Metabolism.

According to data obtained in animal experiments, orlistat metabolism occurs mainly in the walls of the gastrointestinal tract. Approximately 42% of the minimal fraction of the drug that is systemically absorbed in obese patients is accounted for by two main metabolites - M1 and M3.

Molecules M1 and M3 have an open b-lactone ring and inhibit lipase very weakly (1000 and 2500 times less than orlistat, respectively). Given this low inhibitory activity and low plasma concentrations (average 26 ng/ml and 108 ng/ml, respectively) after therapeutic doses, these metabolites are considered pharmacologically inactive.

The main route of elimination is excretion of the drug, not absorbed, in the feces. About 97% of the administered dose of the drug was excreted in the feces, with 83% being excreted as unchanged orlistat.

The cumulative renal excretion of all substances structurally related to orlistat was less than 2% of the administered dose. The time until the drug is completely eliminated from the body (with feces and urine) is 3-5 days. The ratio of orlistat elimination routes in volunteers with normal and overweight was the same. Both orlistat and metabolites M1 and M3 can be excreted in bile.

Indications.

Therapy in combination with a moderately hypocaloric diet in obese patients with a body mass index (BMI) ≥ 30 kg/m2 or overweight patients (BMI ≥ 28 kg/m2), including those who have risk factors associated with obesity.

Treatment with orlistat should be discontinued after 12 weeks if there is no reduction in body weight by at least 5% compared to initial body weight.

Contraindications.

Chronic malabsorption syndrome, cholestasis, hypersensitivity to the active substance or to any other components of the drug, breastfeeding period.

Interaction with other drugs and other types of interactions.

Cyclosporine.

When taking Xenical® and cyclosporine simultaneously, a decrease in plasma concentrations of cyclosporine was observed. This may lead to a decrease in the immunosuppressive effectiveness of cyclosporine. Therefore, simultaneous administration with cyclosporine is not recommended (see Section "Peculiarities of use"). However, if it is impossible to avoid the simultaneous use of orlistat and cyclosporine, it is recommended to more often determine the concentration of cyclosporine in the blood plasma after the administration of orlistat and after its discontinuation in patients receiving cyclosporine. The concentration of cyclosporine in the blood plasma must be monitored until it stabilizes.

Acarbose. In the absence of pharmacokinetic interaction studies, concomitant use of orlistat and acarbose should be avoided.

Anticoagulants for oral administration.

When taking warfarin or other anticoagulants with Xenical®, it is necessary to monitor the international normalization ratio.

Fat-soluble vitamins.

When taken simultaneously with Xenical®, there was a decrease in the absorption of fat-soluble vitamins A, D, E, K. In the vast majority of patients who took part in clinical studies for up to 4 full years of treatment with orlistat, levels of vitamins A, D, E, K and beta carotene remained within the normal range. To ensure proper nutrition, patients on a weight control diet should be advised to include plenty of fruits and vegetables and take multivitamin supplements. If the patient has been recommended multivitamins, they should be taken at least 2:00 after using Xenical® or before bedtime.

Amiodarone.

With the simultaneous use of Xenical and amiodarone, a slight decrease in the level of amiodarone in the blood plasma was observed in a limited number of healthy volunteers. In patients receiving amiodarone, the clinical significance of this phenomenon is unclear, but in some cases it may be clinically significant. Increased clinical and ECG monitoring is recommended in patients receiving orlistat and amiodarone concomitantly.

With the simultaneous use of orlistat and anticonvulsants (valproate, lamotrigine), there have been reports of seizures. A causal relationship has not been established, but patients should be monitored for possible changes in frequency and/or severity.

Rarely, hypothyroidism and/or decreased control of hypothyroidism may occur. The mechanism of this phenomenon has not been proven, however, there may be a decrease in the absorption of iodine salts and / or levothyroxine (see Section “Peculiarities of application”).

Isolated cases of reduced efficacy of HIV antiretroviral drugs, antidepressants and antipsychotics (including lithium) have been reported coinciding with the initiation of orlistat therapy in patients with well-controlled disease. That is why all possible consequences in such patients should be carefully considered before starting treatment with orlistat.

Lack of interaction.

In pharmacokinetic studies with amitriptyline, atorvastatin, biguanides, fibrates, fluoxetine, pravastatin, losartan, phentermine, digoxin, phenytoin, nifedipine GITS (gastrointestinal therapeutic system) and nifedipine slow release. Interaction with oral contraceptives and orlistat has not been demonstrated in specific interaction studies. The possibility of adverse reactions from the gastrointestinal tract may increase if Xenical® is used on a diet rich in fat (for example, with a diet of 2000 kcal/day, more than 30% will be calories of fat origin, approximately 67 g of fat). The daily amount of fat should be divided into three main meals.

Cases of rectal bleeding have been reported when using Xenical®. If severe and/or persistent symptoms occur, further evaluation is recommended.
