Categories Z00-Z99 are intended for those cases when, as
"diagnosis" or "problem" does not indicate illness, injury or external
reason related to sections A00-Y89, and other circumstances.
This situation can arise mainly in two cases.
a) When an individual, not necessarily sick at the time,
applies to a health care institution with any special
purpose, for example, to receive insignificant assistance
or for maintenance due to a current condition: as
organ or tissue donor, for prophylactic vaccination or
discussing a problem that is not itself caused by the disease
or injury.
b) When there are some circumstances or problems that affect health, but at the moment are not themselves a disease or injury. Such factors can be discovered during population health surveys, during which the individual may be sick or healthy; they also
may be recorded as additional circumstances that should be kept in mind when seeking help for any illness or injury.

1. Registration form No. 131/у « A medical examination (preventive medical examination) record card (hereinafter referred to as the Card) is filled out for each patient who applied to a medical organization providing medical care on an outpatient basis to undergo a medical examination (preventive medical examination) (hereinafter referred to as the Medical examination) in accordance with the order Ministry of Health of the Russian Federation dated December 3, 2012 No. 1006n(registered by the Ministry of Justice of the Russian Federation on April 1, 2013, registration No. 27930) and in conducting a preventive medical examination in accordance with the order of the Ministry of Health of the Russian Federation dated December 6, 2012 No. 1011n “On approval of the Procedure for conducting a preventive medical examination” (registered by the Ministry of Justice of the Russian Federation on December 29, 2012, registration No. 26511).

2. One Card is filled out for each patient, regardless of whether the Medical Examination is carried out by one or more doctors.

3. All records about the patient’s health status are made in the “Medical record of a patient receiving medical care on an outpatient basis” - registration form No. 025/u, approved by order of the Ministry of Health of Russia dated No. (registered by the Ministry of Justice of the Russian Federation in February 2015, registration number) (hereinafter referred to as the Medical Card): complaints, anamnesis, objective data, diagnoses: main, background, competing and concomitant diseases, injuries, poisonings with their ICD-10 codes, health group, prescribed treatment, examination, establishment of dispensary observation, as well as the results of examination and follow-up.

4. Information about the clinical examination necessary for the preparation of state statistical reporting is also entered into the “Coupon of a patient receiving medical care on an outpatient basis” - registration form No. 025-1/u, approved by order of the Ministry of Health of Russia dated No. (registered by the Ministry of Justice of the Russian Federation in February 2015, registration No.) (hereinafter referred to as the Coupon).

5. In paragraph 5 of the Card, benefit category codes are entered in accordance with the categories of citizens entitled to receive state social assistance in the form of set of social services 1:

“1” - war invalids;

“2” - participants in the Great Patriotic War;

“3” - combat veterans from among the persons specified in subparagraphs 1-4 of paragraph 1 of Article 3 of the Federal Law of January 12, 1995 No. 5-FZ “On Veterans” 2;

“4” - military personnel who served in military units, institutions, military educational institutions that were not part of the active army, in the period from June 22, 1941 to September 3, 1945 for at least six months, military personnel awarded orders or medals USSR for service during the specified period;

“5” - persons awarded the badge “Resident of besieged Leningrad”;

“6” - persons who worked during the Great Patriotic War at air defense facilities, local air defense facilities, in the construction of defensive structures, naval bases, airfields and other military facilities within the rear borders of active fronts, operational zones of active fleets, at front lines sections of railways and highways, as well as crew members of transport fleet ships interned at the beginning of the Great Patriotic War in the ports of other states;

“7” - family members of deceased (deceased) war invalids, participants in the Great Patriotic War and combat veterans, family members of persons killed in the Great Patriotic War from among the personnel of self-defense groups of facility and emergency teams of local air defense, as well as family members of deceased workers hospitals and clinics of the city of Leningrad;

“8” - disabled people;

"9" - disabled children.

In paragraph 9 of the Card, the dates of inspections and examinations for the first stage of Medical Examination are noted according to the age schedule, as well as the identification of deviations based on the results of each type of examination and examination. In the “Notes” column of paragraph 6, the date of the study that was offset from previously conducted or the refusal of the study is noted, which makes it possible for an expert assessment of the completeness of the 1st stage of the Medical Examination.
9. In paragraph 10 of the Card, the dates of inspections and examinations at the second stage of Medical Examination are noted according to the indications identified at the first stage, as well as the identification of deviations based on the results of each type of examination and examination. Indications for the methods of the second stage of clinical examination:

9.1. D complex scanning brachycephalic arteries:

Based on the results of the questionnaire, there is an indication or suspicion of a previously suffered acute cerebrovascular accident, or

For men aged 45 years and older and women aged over 55 years with a combination of three risk factors: high blood pressure, dyslipidemia, overweight or obesity;

9.2. Esophagogastroduodenoscopy:

Based on the results of the survey, identification of complaints indicating a possible oncological disease of the upper gastrointestinal tract, or

For citizens over the age of 50 with a family history of cancer of the gastrointestinal tract);

9.3. Examination (consultation) with a neurologist: based on the results of a questionnaire in the case of indication or suspicion of a previously suffered acute cerebrovascular accident in citizens who are not under clinical observation in this regard, as well as in cases of primary detection of motor dysfunction, cognitive impairment and suspicion of depression in persons 75 years of age and older;

9.4. Examination (consultation) with a surgeon or urologist: for men aged 42 to 69 years with signs of pathology of the genitourinary system first identified by the results of a questionnaire or with a family history of prostate cancer, as well as for patients with suspicion of prostate cancer identified based on the results of an ultrasound examination);

9.5. Examination (consultation) with a surgeon or coloproctologist for citizens with a positive stool test for occult blood, with a family history of polyposis, colorectal cancer, with other indications identified based on the results of a questionnaire, as prescribed by a general practitioner, urologist, obstetrician-gynecologist in cases of detection of symptoms of a malignant neoplasm of the colorectal regions;

9.6. Colonoscopy or sigmoidoscopy: if there is a suspicion of cancer as prescribed by a surgeon or coloproctologist;

9.7. Determination of blood lipid spectrum(level of total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, triglycerides): for citizens with a detected increase in the level of total cholesterol in the blood);

9.8.Spirometry: for persons with suspected chronic bronchopulmonary disease based on the results of a questionnaire, smokers and on the referral of a general practitioner;

9.9. Examination (consultation) with an obstetrician-gynecologist: for women with identified pathological changes based on the results of a cytological examination of a cervical smear and (or) mammography, and/or according to the results of an ultrasound scan of the uterus and ovaries;

9.10. Determination of the concentration of glycated hemoglobin in the blood or glucose tolerance test: for citizens with a detected increase in blood glucose levels;

9.11. ABOUTexamination (consultation) with an otorhinolaryngologist: for citizens aged 75 years and older, if there are indications based on the results of a questionnaire or examination by a general practitioner;

9.12. Blood test for prostate-specific antigen levels: as prescribed by a surgeon or urologist for men with suspected prostate cancer based on the results of a survey, examination, digital examination of the prostate and/or ultrasound of the prostate;

9.13. Examination (consultation) with an ophthalmologist: for citizens aged 39 years and older who have increased intraocular pressure and for citizens aged 75 years and older who have decreased visual acuity that cannot be corrected with glasses, identified by the results of a survey (questionnaire);

9.14. Individual in-depth preventive counseling or group preventive counseling (patient school): for citizens with identified risk factors for chronic non-communicable diseases, as well as those with diseases or high and very high total cardiovascular risk.

9.15. Appointment (examination) with a general practitioner, including establishment (clarification) of the diagnosis, determination (clarification) of the health group, determination of the dispensary observation group and the physical activity group (taking into account the opinions of medical specialists), as well as the referral of citizens, if there are medical indications, for an additional examination that is not included in the medical examination program, to receive specialized, including high-tech, medical care, and for sanatorium and resort treatment.

In the “Notes” column of paragraph 10, the date of the study that was offset from previously conducted or the refusal of the study is noted, which makes it possible for an expert assessment of the completeness of the 1st stage of the Medical Examination.

10. In paragraph 11, in the lines with the names of classes and individual diseases, the dates are noted: in column 4 - identification (confirmation) during a clinical examination of diseases, in column 5 - identification of a disease, including one diagnosed for the first time in life based on the results of a clinical examination. The source of information is paragraph 20 “List of updated diagnoses” of the Medical Card.

Column 6 of clause 11 records the admission for dispensary observation regarding the identified disease (including those diagnosed for the first time).

In column 7 of paragraph 11, suspicion of diseases is registered, the list of which is given in column 1.

11. Paragraph 12 records risk factors for chronic non-communicable diseases. In the corresponding column, under the indication of the risk factor code according to ICD-10, the date of identification of the following risk factors is noted.
