Wobenzym is a preparation for oral administration containing lytic enzymes, mainly proteolytic ones - bromelain, papain, trypsin, chymotrypsin in combination with rutoside.

A mixture of enzymes, including bromelain and papain, helps to reduce edema of traumatic origin by enzymatic proteolysis in the interstitium of macromolecules of inflammatory mediators (histamine, etc.). In addition, microcirculation improves in the area of ​​damage due to lysis of fibrin and microthrombi, which helps to reduce edema. In laboratory animals (rats), the ability of the proteolytic enzymes that make up the Wobenzym preparation to inhibit platelet aggregation was demonstrated. In experimental and clinical models, the regulatory effect on the immune system of a mixture of proteases included in the preparation Wobenzym has been demonstrated.

Rutoside is a glycoside of the flavonoid quercetin, has P-vitamin activity. Helps reduce capillary permeability and fragility.

Pharmacokinetics

The mechanism of absorption of macromolecular substances, such as enzymes, by paracellular diffusion in the intestinal mucosa is described. There is also a breakdown of proteases in the intestine to amino acids. For bromelain and papain, the half-life is 5.5-8 hours and 36-48 hours, respectively. Rutoside appears to be rapidly metabolized after oral administration. Quercetin metabolites are determined in the blood: 3,4-dihydroxy-phenylacetic acid, 3,4-dihydroxytoluene, 3-hydroxy-phenylacetic acid and 3-methoxy-4-hydroxy-phenylacetic acid (homovanillic acid). After oral administration of rutoside, about 50% of its metabolites are excreted from the body within 48 hours in the urine.

Indications for use

Wobenzym is used as part of the complex therapy of inflammation, swelling and pain in:

trauma; thrombophlebitis; arthritis, including rheumatoid arthritis, and arthrosis.

Contraindications

In case of hypersensitivity to active substances or to one of the other components; patients suffering from congenital or acquired blood clotting disorders, such as hemophilia; patients with severe liver and / or kidney disease; patients immediately prior to surgery

Use during pregnancy and lactation

Pregnancy and lactation are not a contraindication for the use of the drug, however, pregnant women should take Wobenzym under the supervision of a physician.

Dosage and administration

Tablets should be taken orally, at least 30 minutes before a meal or 90 minutes after a meal, without chewing, with water (200 ml).

Depending on the activity and severity of the disease, Wobenzym is taken at a dose of 3 to 10 tablets 3 times a day. In the first three days of taking the drug, the recommended dose is 3 tablets 3 times a day.

With an average activity of the disease, Wobenzym is prescribed at a dose of 5-7 tablets 3 times a day for 2 weeks. In the future, the dose of Wobenzym should be reduced to 3-5 tablets 3 times a day, a course of 2 weeks.

With high disease activity, Wobenzym is prescribed at a dose of 7-10 tablets 3 times a day for 2-3 weeks. In the future, the dose of Wobenzym is reduced to 5 tablets 3 times a day, the course is 2-3 months.

In chronic long-term diseases, Wobenzym can be used according to indications in courses from 3 to 6 months or more.

The daily dose of Wobenzym should not exceed 30 tablets.

Treatment, as a rule, continues until the symptoms of the disease decrease.

Separate groups of patients

Dosage modification is not required in patients with mild or moderate hepatic and renal impairment. Wobenzym should not be taken in patients with severe hepatic and/or renal impairment.

In the elderly, a change in dosage is not required.

Children

There are no sufficient studies regarding the effect of the drug when taken by children and adolescents under 18 years of age with these indications.

Side effects

Wobenzym is well tolerated by patients. In most cases, side effects, withdrawal syndrome, addiction were not observed even with long-term treatment with high doses. In some cases, there are slight changes in the consistency and smell of feces, skin rashes in the form of urticaria, which disappear with a decrease in the dose of the drug or withdrawal. If other adverse reactions occur that are not noted in the instructions, it is recommended to stop taking the drug and consult a doctor.

Interaction with other drugs

With the simultaneous use of the drug Wobenzym and antibiotics, the level of antibiotics in plasma and urine, in particular, tetracyclines, sulfamids and amoxicillins, is increased.

Co-administration of Wobenzym with anticoagulants or inhibitors of platelet aggregation may increase the slowing down of blood clotting. Therefore, these medicines should be taken with Wobenzym strictly as directed and under close supervision.
